Merakey is now hiring for a full time LPN Residential Mental Health (overnight, 4 shifts/wk) for our Russelton, PA location.
**No on-call duties required!!
The nursing position (LPN) is responsible for completing and monitoring all medical requirements of the program.
Adult Behavioral Health Vision Statement: Merakey Adult Behavioral Health Services support individuals in attaining wellness and recovery through respectful, person-centered relationships and services. These services will promote hope and empowerment to help individuals reach their full potential.
Position Specific Responsibilities: 1. Administer medication and therapeutic treatments as per physician’s orders and Merakey policies and procedures. 2. Provide ongoing assessment of mental and medical status to include, vitals, weights, change in mental status, side effects/allergic reactions, etc. 3. Provide care in a manner that considers the patients’ rights, safety, comfort, and the overall therapeutic environment. 4. Provide individualized education to residents on relevant health topics. 5. Assess and develop resident medication education treatment goals. 6. Provide documentation relative to patient situations and conditions reflecting of the treatment plans 7. Participate in continuous quality improvement. 8. Communicate with co-workers and supervisors in timely manner of situations that affect patient care or well-being. 9. Assists in the orientation/training of new staff members. 10. Understand and implement policies and procedures relating to medication administration, daily routines, client care and rights, safety management, and unusual incidents. 11. Complete routine medical assessments on each client based off individual needs. 12. Follow treatment plans. 13. Complete nursing portion of discharge summaries. 14. Act as a liaison between resident and outside healthcare provider to ensure continuity of care. 15. Contact individual pharmacies to authorize medication refills on any client related issues and collaborate with the psychiatrist. 16. Participate in weekly treatment team meeting. 17. Communicate resident health related concerns to all staff in an effective manner. 18. Provide in-services on medical topic as directed. 19. Maintain a clean and sanitary work environment. 20. Education: Graduate of an accredited program and possess a valid PA Registered license.
Work Experience: 1-year paid work experience with behavioral health, forensic, sexual offending behaviors or residential services, RN preferred Knowledge and understanding of severe mental illness, recovery principles and recovery driven service system
Other: Valid Driver’s License required All Clearances required by OMHSAS regulations
